We installed between a metal platform and mattress. I don't know if between a mattress and box spring would be any closer. However, there is a 2 to 3 inch gap between the mattress and the railing. Which on two occasions became a problem as the sleeper became wedged in the gap causing some significant bruising. We cut up a pool noodle and used duct tape to eliminate the gap and it is working well. The swing gate is very useful to assist in rising.
